• No Substances: Bringing alcohol, drugs, or weapons to the facility is strictly prohibited and grounds for discharge

4. Payment & Billing

Financial responsibilities include:

• Insurance: We will bill your insurance company for covered services. You are responsible for deductibles, copayments, and non-covered services
• Private Pay: Payment is due at time of service or according to agreed payment plan
• Outstanding Balances: Unpaid balances may be sent to collections
• Refund Policy: Deposits are non-refundable. Services provided are non-refundable
• Insurance Changes: You must notify us immediately of insurance changes or loss of coverage
